The 2017 OFA Annual General Meeting will be held on December 2, 2017 at the Markham Pan Am Sports Centre.

Nomination Form
Members are requested to confirm their attendance, return Proxy Form, and Nomination Form by November 25th. This will provide time to process the papers in preparation for the AGM. Registration and the distribution of voting papers will begin at 10:30am.
Instructions for those attending the AGM
1. OFA list of Paid Clubs
Clubs are encouraged to confirm that their membership payments have been registered. If you would like confirmation of your membership status, and the number of registered athletes attached to your club (competitive and recreational), please contact the OFA Secretary by November 25th.
2. Use of a Proxy and Authorizations
A paying Club member is entitled to participate and make use of his or her rights as a member at the Annual General Meeting by proxy.
A person holding a shareholder’s proxy should be in possession of a dated letter of proxy form (attached) to demonstrate that he or she is entitled to act in this capacity on behalf of a member.
Originals of members’ letters of proxy should be sent to OFA Secretary as instructed on the form, to reach the OFA by November 25th.
3. Board Nomination Form
A Board Nomination form has been attached and interested members are requested to return their completed form to the OFA Secretary by November 25th. Two positions are up for nomination this year:
